IMAGES 1. Throughout the program parents and Outer Harbour Sailing Federation staff takes photos and video of the participants. These images may be displayed on the club website and promotional materials or in local newspapers. Unless otherwise stated you consent to the use of images as indicated above. RELEASE 2. In consideration of acceptance of this application for the above named applicant, I agree to save harmless and keep indemnified the Outer Harbour Sailing Federation, their clubs, their directors, employees, instructors, contractors, volunteers and members from and against all claims, actions, costs, expenses and demands in respect to death, injury, loss or damages to the participants person or property, howsoever caused which arises out of or in connection with his/her taking part in this program. I further understand that this release is binding upon myself, my heirs, executors and assigns. TERMS & AGREEMENTS 3. I agree and acknowledge that my child will participate in water sports and related activities entirely at her/his own risk and that she/he is medically fit to undertake such activities. 4. I the parent/guardian am assuming full responsibility for the applicant’s health being such that the activities will in no way aggravate any condition that is present. It is assumed the parent/guardian will know the child’s condition and seek medical advice before participating. The parent/guardian will notify the Program Director if for any reason this permission is changed or withdrawn. 5. I (and my parent/legal guardian if I am under 18) confirm that I have reviewed the provided Concussion Resources at https://watersport.camp/safety and I acknowledge and commit to the Ontario Sailing Concussion Code of Conduct. 6. All refunds will be less a 3.5% processing and $50 administration fee. This includes refunds if camp is cancelled because of COVID-19 restrictions or the COVID-19 situation. I acknowledge that should I notify the Program Director to cancel this application a minimum of thirty business days prior to the first day of the program a refund minus a 3.5% processing and $50.00 administration fee will be granted. If cancellation is requested in less than thirty days prior to the start of the program with a valid medical or relocation reason, a 50% refund will be issued. If no notice is received prior to the start of the program, no refund will be issued. All cancellation requests must be received in writing. 7. I agree that core contact information will be shared with Ontario Sailing, Sail Canada, Ontario Rowing, Rowing Canada and Governments that require statistical information for funding purposes. Our clubs/school, Ontario Sailing, Sail Canada, Ontario Rowing and Rowing Canada will not sell, lease, rent or use this contact information for any commercial purposes. Those who become Training Members through the Water Sports Program will not receive any promotional material from any of Ontario Sailing’s or Ontario Rowing’s sponsors/partners. This will supersede section 3.3(0) of the Ontario Sailing Privacy Policy.
